Certification required: The Travel Consent Form must be certified by a UK solicitor or notary public before it can be apostilled.
Original document: The apostille will be applied to the certified original.
Not eligible for e-apostille: This document must go through the physical apostille process.
Each adult providing consent (e.g., both parents or legal guardians) must sign the form in front of a solicitor or notary.
The solicitor or notary must confirm the identity of the signatories and certify the document accordingly.
Some countries may require the consent form to be translated and legalised by their own embassy after the UK apostille. Always check the specific requirements of the destination country.
